Pathways Case Manager

Full-time, Pay DOE

Job Summary: 

This position is responsible for case management for the Literacy Council of Alaska’s Pathways Program under the Workforce Innovation and Opportunity Act Youth (WIOA-Y) grant for eligible, out of school, participants aged 16-24. This includes providing services and assisting participants in overcome barriers, and creating education and employment goals. Case Managers track progress, assist participants with meeting education and career goals, and coordinate with partners in the community who provide employment and supportive services as needed.

Qualifications: 
  • Associate’s Degree or equivalent (1-year experience in case management may be substituted).

  • Proficient in the use of Microsoft Office.

  • Adept at working independently as well as with team members.

  • Hold excellent customer service skills.

  • Demonstrate effective oral and written communication skills.

  • Able to organize and prioritize a variety of tasks.

  • Work with minimal supervision.

  • Able to coordinate multiple tasks and follow-up in spite of frequent interruptions.

  • Maintain a professional attitude with enthusiasm and dependability.

  • Exercise time management skills.

  • Willing to take on new tasks with an eagerness to learn new skills.

Job Tasks: 
  • Assess participants for financial, employment, educational/training, family composition and status, support network, and other information to identify challenges and barriers to self-sufficiency.

  • Communicates with participants as required.

  • Develop individual plan to direct the activities of participants in moving toward economic self-sufficiency.

  • Monitor participants to ensure they are achieving goals that lead to employment.

  • Establish, maintain, and review participant files as well as prepare data for required reports accurately and in a timely manner.

  • Identify and appropriately manage participants in crisis.

  • Refer participants to appropriate community resources for services that address and meet specific needs as identified.

  • Refer participants to training projects that will lead to a higher level of self-sufficiency.

  • Prepare for and attend case status updates and meetings with participants.

  • Participate as a team member in weekly program staff meetings and other meetings as required.

  • Works with other organizations’ staff as a team to collaborate and meet the needs of all participants.

  • Develop employment and training opportunities and make appropriate job, education, and training matches and referrals for participants.

  • Other duties as assigned.
